Position Overview
This role focuses on patient care coordination, case management, compliance, and supporting positive patient outcomes in a home health setting.
Responsibilities
- Open and close patient cases
- Conduct patient assessments and care coordination
- Maintain compliant documentation and records
- Collaborate with interdisciplinary care teams
- Participate in on-call rotation as needed
Qualifications
- Active RN license required
- Home Health experience required
- Strong documentation and case management skills
- Ability to work independently in the field
